Additional Canadian Privacy Rights

If you reside in Canada, you also have the following additional rights with respect to your personal information:

• To request access to personal information collected about you;
• To correct the personal information we hold about you; and
• In certain circumstances, to withdraw your consent to the use of your personal information.

You may submit a request to exercise your rights to access, correct or delete your Personal Information through one of two means:

2. By calling us at 1-888-408-1115.

We may not accommodate a request if we believe it may violate a law or legal requirement, or if a request to change your personal information may cause it to be incorrect. In order to process your request, we must verify your identity. We may do this by asking you to:

• provide personal identifiers that we can match against information we may have collected from you previously; 
• confirm your request using the email account or telephone number stated in the request; and/or
• providing additional information such as a signed declaration
